What the forms collect

If you use the appointment, enquiry or feedback form, we collect the details you enter: your name, phone number, email address (if you give one), the service you are asking about, your preferred time to be called, and any message you write.

What we use it for

Only to respond to you and to improve the website. Your details are not published, sold or shared for advertising. A message you send us is never used as a testimonial without your separate consent.

Where it is kept

Submissions are saved in private storage and can be read only by clinic staff who sign in with a password. They may also be emailed to the clinic.

How long we keep it

For as long as we need to respond to you. You can ask us to delete your submission at any time by writing to doctor@unisonmedicare.com.

Cookies and tracking

This website does not use advertising or tracking cookies. We count how many people tap the call, WhatsApp, email and directions links so we can see what is useful. These counts do not identify you.

Please do not send medical reports through forms, email or WhatsApp

Do not send urgent or highly sensitive medical information by email or WhatsApp. Bring reports to your visit, or call us and we will tell you the safest way to share them.
